By purchasing, accepting or using an Antidote Gift of Good Health (the "Antidote Gift") you agree to be bound by these Gift Terms and Conditions, Antidote Terms of Use and Privacy Policy.  

Offer Period: 

Gift coupons at the promotional price are available for a limited time only, for as long as it is advertised on www.antidotehealth.com as stated on the promotion coupon or otherwise advised.

Unique Gift Coupon Codes:

Each Gift coupon code is unique and may only be exchanged one time for telehealth services. 

Purchaser may send the Gift of Good Health Coupon to one receiver only.

To ensure that the gift coupon code remains active and redeemable, keep it secret and do not let anyone else use it or abuse it (e.g., don't post it on social media or any other platform). 

If Antidote Health learns that the gift coupon code has been misused in any way (e.g., selling it or sharing it with others), we reserve the right to cancel your gift coupon code and/or suspend or close your account without informing you.

Redemption

Receiver may redeem the Antidote Gift by creating an Antidote Health account and being qualified to receive services.

To redeem the Antidote Gift, enter the gift coupon code into the "Use Coupon" link during the checkout process.

Each Antidote Gift is redeemable for one consultation with an Antidote clinician.

Gift coupons cannot be applied to previous purchases nor combined with any other offers or discounts.

The right to use Antidote Gift by the receiver does not expire and no maintenance fees apply.

Qualification

Receiver is located (during the consultation) in a state where Antidote Health provides services (currently: AZ, CA, CO, DC, FL, GA, IL, MA, MD, MI, MO, NC, NJ, NY, OH, PA, TX, UT, VA, WA and WI).

Receiver is not be a Medicare or Medicaid beneficiary. This includes Medicare Advantage, Medicare Part D, Medicaid managed care, or Children’s Health Insurance Program (CHIP) plan. 

Restrictions

Antidote Gift is not refundable or redeemable for cash or credit, unless special circumstances apply or as required by law.

Antidote Gift may only be used for a one time visit from Antidote Health.

Resale of Antidote Gift is strictly prohibited. 

Refund

If it is determined that the recipient does not qualify for services either due to location, Medicare or Medicaid eligibility or any other qualification issue, or if recipient declines accepting the gift, the recipient should contact Antidote Health Customer Support at support@antidotehealth.ai and request a refund to the Antidote Gift purchaser. 

Changes: Antidote Health reserves the right to cancel the promotion price for gift coupons, modify the gift coupon Terms and Conditions, or change any specific promotion terms at any time and in its sole discretion.
